29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 24, 2017, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) Throughout the shop, at least five chip conveyors being used without a guard provided to prevent employees contacting moving parts while in operation. 2) In the shop, at least two CNC machines were found not having interlock device provided on door to prevent employees contacting moving parts while in operation. Employees were exposed to possible amputation and crushing hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about May 24, 2017, and at times prior thereto, at the Mazatrol Cam T-2 CNC lathe machine a chip conveyor driven by a sprocket wheel and chain system less than 7 feet from the floor were not enclosed. Employees were exposed to an amputation and caught in hazards.
