29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  The employer is failing to ensure that the training provided to forklift operators comprised the required formal instruction and evaluation.  Employees were exposed to struck-by hazards.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:  In the area of the gin near the location where cotton is fed into the gin the employer did not ensure that the AWG 16/3 SJTW extension cord providing power to the tarp roller was used in one continuous length without a splice. Employees were exposed to a shock hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  In the area of the gin near the location where cotton is fed into the gin the employer did not ensure that the AWG 16/3 SJTW extension cord providing power to the tarp roller had the ends of the cord firmly attached to the male and female ends. Employees were exposed to a shock hazard.
