29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:  Location: 95-111 Rail Road Ave., Newark, NJ.  a) Employees were exposed to a potential fall hazard since the tubular welded scaffold's platform the employees were using was missing the midrail at an edge that had a height of approximately 16 ft to the ground below; on or about 5/19/17.
