29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) on the jobsite - employees working on a 5 in 12 sloped roof were exposed to a 9ft 8-1/4in fall hazard, in that, fall protection was not used, on or about April 10, 2017.  (b)  on the jobsite - employees working on a 5 in 12 sloped roof were exposed to a 9ft 8-1/4in fall hazard, in that, fall protection was not used, on or about April 12, 2017.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) on the jobsite - a portable extension ladder used to access the roof was extended approximately 18 inches above the roof landing when the ladder's length allowed for it to be extended further to at least 3 feet, exposing employees to a fall hazard of up to 9ft 8-1/4in (eave height), on or about April 12, 2017.
