29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard. On or about May 30, 2017 employees were exposed to fall hazards while ascending and descending stairs with 5 risers that were not equipped with a standard stair railing to access the bin 12 auger and tunnel from the dump pit. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings. On or about May 30, 2017 employees were exposed to fall hazards while ascending and descending stairs with a missing top tread. This condition required employees to make a step twice the riser height of the other risers in the stair used to access the bin 12 auger and tunnel from the dump pit. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about May 31, 2017 employees were exposed fall hazards of at least 32 feet while moving from a catwalk platform with a gap in the guardrail system that did not join the catwalk platform to a bin roof ladder guardrail system. This catwalk and bin were located south and west of the grain dryer. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about May 31, 2017 employees were exposed fall hazards of at least 32 feet from a platform floor hole of approximately 1 foot due to an improperly fitted catwalk platform that joined a grain bin roof ladder. The catwalk and bin were located west of the grain dryer. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
