29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about May 31, 2017, the Krauss Maffei injection molding machine was under repair or maintenance and had not been locked out by the authorized employee. Employees exposed to the hazard of being struck-by or caught-in the machine, or receiving and electrical shock. The Prime Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 1059373, citation number 1, item number 2, and was affirmed as a final order on October 16, 2015, with respect to a workplace located at 407 E Hwy US 90, Schulenburg, TX 78956.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about May 31, 2017, in the maintenance department, at the Baldor pedestal grinder the distance between the abrasive wheel periphery(s) and the adjustable tongue exceeded the one fourth inch, exposing employees to the hazards of being struck-by flying wheel fragments in the event of wheel breakage. The Prime Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1059373, citation number 1, item number 3b, and was affirmed as a final order on October 16, 2015, with respect to a workplace located at 407 E Hwy US 90, Schulenburg, TX 78956.
