29 CFR 1910.28(b)(1)(i) Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i). On or about February 13, 2024, and at times prior thereto, employees were exposed to a fall hazard while repairing portions of the roof that had leaks at heights of approximately 10-22 feet without the use of fall protection.

29 CFR 1904.32(a)(2): At the end of each calendar year an annual summary of injuries and illnesses recorded on the OSHA 300 log was not completed in detail. On or about February 13, 2024, and at times prior thereto, the employer did not ensure that the OSHA Form 300A Summary of Work-Related Injuries and Illnesses for the years 2022 and 2023, accurately reflected the cases in columns (H) and (I) and the number of injury types under column (M).
