Employees perform maintenance and mold changes on the injection molding machines. Employees were exposed to the hazard of being caught-in the machine and/or an electrical shock hazard from an unexpected start-up of the machine. Energy control procedures were not developed for such machines as, but not limited to:    1. Sumitomo Demag SE385EV-A, injection molding machine. Some potential energy sources for this machine are electrical, hydraulic and pneumatic.  2. Sumitomo SE180E, injection molding machine. Some potential energy sources for this machine are electrical, hydraulic and pneumatic.

The internal conductors were pulled out of their outer sheath and wired together. Employees working near the unguarded chain and sprockets when picking up plastic cups and packing them. Employees exposed to the hazard of being caught-in the rotating chain and sprockets.  The Prime Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(f)(3), which was contained in OSHA inspection number 1059373, citation number 1, item number 4, and was affirmed as a final order on October 16, 2015, with respect to a workplace located at 407 E Hwy US 90, Schulenburg, TX 78956.
