This was most recently documented on or about 5/31/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas. Employees working in close proximity to powered industrial trucks throughout the facility were exposed to struck by hazards in that all nameplates and markings were not in place on the following forklifts:    (b) Nissan 40 - data plate is damaged and not illegible and the Direction Control Lever is not labeled.

Employees engaged in maintenance activities using the horizontal band saw in the maintenance shop were exposed to amputation hazards in that approximately 4 inches of the unused portion of the blade was not guarded.

Employees engaged in maintenance activities using the Bridgeport Mill, serial number J-120120, in the maintenance shop were exposed to caught in hazards in that the housing guards covering the step pulleys, less than 7 feet above the ground level, were missing.

This was most recently documented on 06/07/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas.   a) Employees in the production area were exposed to electrical shock hazards while operating the eDart system used with the #105 Cincinnati Milacron injection mold press machine. The eDart system was powered by an extension drop cord  with a missing ground pin.  b) Employees in the Maintenance Shop area were exposed to electrical shock hazards while operating the horizontal band saw machine. The cord supplying power was being used with damaged exposed wiring.

This was most recently documented on 06/07/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas.   a) Employees engaged in production activities, using the #105 Cincinnati Milacron injection mold press, in the production area were exposed to electrical shock hazards in that the flexible cord supplying power to the press was not equipped with a ground pin.  b) Employees in the Maintenance Shop area were exposed to electrical shock hazards while operating the horizontal band saw machine. The cord supplying power was being used with damaged exposed wiring.

This was most recently documented on or about 5/31/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas. Employees in and around operating forklifts were exposed struck-by hazards in that the employer did not have a manufacturers owner's manual available to provide truck specific training.
