On or about May 30, 2017, located throughout the facility at TOSOH SMD, Inc., the employer did not ensure that machine specific procedures were developed, documented and utilized for employees performing service and maintenance activities, i.e. preventive and breakdown maintenance, tool changes, testing, troubleshooting, etc. on approximately 159 machines, i.e. Mazak CNC machines, thereby exposing the employees to the release of stored energy and the unexpected energization of machines and equipment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.       a. On or about May 30, 2017, employees bypassed safety interlocks on a Mazak E410HS-01 in order to clean and buff metal material while their hand(s) were within less than one to four inches from the rotating chuck of the machine, thereby exposing the employees to a caught-in hazard.       b. On or about May 30, 2017, a device was used to depress control buttons to override interlock functions of a sliding door on a Mazak 500 CNC machine allowing the chuck to continue to rotate so that employees could clean and buff metal material, thereby exposing the employees to a caught-in hazard.      c. On or about May 30, 2017, two heavy duty nuts with two bungee cords were used to depress control buttons to override interlock functions of a sliding door on a Mazak YB50-02 CNC machine allowing the chuck to continue to rotate so that employees could clean and buff metal material, thereby exposing the employees to a caught-in hazard.
