29 CFR 1910.28(b)(3)(i): 29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a. On or about May 28, 2025 and times prior, located at 3600 Gantz Road, Grove City, Ohio 43123 the employer did not ensure that employees were protected through any hole or opening that was 4 feet or more above the lower level. Employees were exposed to trips and falls when transitioning onto the supported scaffold from the concrete walking-working surface where there was a 12 inch by 40 inch gap at Press 1 (Press HYD005) in Building 4 of the facility.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a. On or about May 28, 2025 and times prior, located at 3600 Gantz Road, Grove City, Ohio 43123 the employer did not ensure that the scaffold components situated at Press 1 (Press HYD005) in Building 4, were inspected for visible defects by a competent person before each work shift to verify the scaffold's structural integrity. Employees utilized the scaffold to access the teach pendant for the ABB robot required to conduct the qualification process. The condition exposed employees to potential injuries in the event of scaffold failure as a result.
