On 5/10/2017, Samuelson Exteriors was installing siding on a residence located at 108 Eagle Estates Drive in Lake St. Louis, MO. A worker was not wearing a protective helmet while working beneath two other workers which were overhead on ladder jack scaffolding. The worker on the ground was subject to head injuries from possible falling tools and supplies.

On 5/10/2017, Samuelson Exteriors was installing siding on a residence located at 108 Eagle Estates Drive in Lake St. Louis, MO. Two workers were working from ladder jack scaffolding on the side of the house approximately twelve to fifteen feet above the ground. The ladder jack scaffolding consisted of three ladders and two platforms. The platforms were bridged together on the center ladder.
