On or about 8/23/2018, Samuelson Exteriors was installing siding on a residence located on lot 52 Carolyn Circle in Wright City, MO.  Two workers were installing siding while working from scaffolding which consisted of ladder jack scaffolding and a six foot portable ladder.  The scaffolding was constructed with ladder jacks positioned on the outsides and a portable ladder in the center to support two scaffold planks.  The portable ladder was not used for purpose for which it was designed.
