29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) OC Company, LTD, at 4002 W 16th Ave, Denver, CO 80204: On and before June 1st, 2017, the employer did not ensure that each employee on a framing operation with unprotected sides and edges 6 feet or more above lower levels was protected from falling. Employees were framing residential townhomes, standing on joists, and were working without fall protection. This condition exposed the employees to a fall hazard of up to approximately 9 feet to a lower level and 18 feet to the ground.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift: Note to paragraph (b)(2)(v). Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. a) OC Company, LTD, at 4002 W 16th Ave, Denver, CO 80204: On or before June 1st, 2017, the employer did not ensure that an employees was using a personal fall arrest system when working from an aerial lift. An employee was performing framing operations from the basket of a Genie aerial lift and was not wearing fall protection. This hazard exposed the employee to a fall of approximately 20 feet.
