29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a)  OC Company LTD, at 10502 Belle Creek Blvd., Henderson, CO 80640: On and before November 20, 2017, the employer did not ensure that employees working on a pump jack scaffold more than 10 feet above a lower level were protected from falling by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  An employee was working on a pump jack scaffold system that did not include guardrails, and no personal fall arrest system was in use.  This condition exposed employees to fall hazards of approximately 12 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or a personal fall arrest system:  (a)  OC Company LTD, at 10502 Belle Creek Blvd., Henderson, CO 80640:  On and before November 20, 2017, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by a guardrail system, safety net system, or a personal fall arrest system.  Employees were working on roofs without the use of fall protection.  Condition exposed employees to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  (a) OC Company LTD, at 10502 Belle Creek Blvd., Henderson, CO 80640:  On and before November 20, 2017, the employer did not ensure that the ladder side rails extended at least 3 feet above the upper landing surface being accessed, where portable ladders were used for access to an upper landing surface and the ladder's length allowed.  Employees used a portable ladder to access a roof at a height of approximately 10 feet.  The ladder was positioned next to the roof against the wall of the structure and did not extend at least three feet above the landing surface.
