29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) OC Company LTD, at 10502 Belle Creek Blvd., Henderson, CO 80640: On and before November 20, 2017, the employer did not ensure that employees working on a pump jack scaffold more than 10 feet above a lower level were protected from falling by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. An employee was working on a pump jack scaffold system that did not include guardrails, and no personal fall arrest system was in use. This condition exposed employees to fall hazards of approximately 12 feet.

Employees used a portable ladder to access a roof at a height of approximately 10 feet. The ladder was positioned next to the roof against the wall of the structure and did not extend at least three feet above the landing surface.
