29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  At the jobsite located 6 West Main Street, Wentzville, Missouri, on or about May 09, 2017, employee(s) for Success Roofing were observed working on the ground where roofers were throwing debris from the roof, without use of head protection.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   At the jobsite located 6 West Main Street, Wentzville, Missouri, on or about May 09, 2017, employee(s) for Success Roofing were observed preparing to torch black board on a low slope roof without the use of fall protection.  The fall distance was approximately 9 feet to the ground below.
