29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  JOB SITE: 663 Berry Wine Lane, Arnold, MO 63010, on or about 10/3/2019, workers were observed on a 8:12 pitch roof, actively installing new roofing material, and were permitted to operate pneumatic nail guns without any form of eye or face protection.  The workers were exposed to struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  JOB SITE: 663 Berry Wine Lane, Arnold, MO  63010, on or about 10/3/2019, workers were observed on top of an 8:12 pitch roof, approximately 9 feet 8 inches above ground level, actively installing new roofing material and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.   SUCCESS ROOFING was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1390497, citation number 1, item number 1, issued on 7/22/2019 and was affirmed as a final order on 8/14/2019 with respect to a workplace located at 5433 Holly Hills Avenue, Saint Louis, MO 63109.  SUCCESS ROOFING was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1236967, citation number 1, item number 2, issued on 7/26/2017 and was affirmed as a final order on 10/26/2017 with respect to a workplace located at 6 W. Main Street, Wentzville, MO 63385.
