On or about 6/6/2017, during residential roofing activities located at 2516 Daegan Dr. Avon OH 44011, the employer failed to ensure that employees tasked with installing roofing materials were provided with their own anchor points. Multiple employees were tied to one anchor point that was rated for one employee, and two employee fall arrest lines were tied to each other compressed between roofing materials without being anchored to the structure.
