29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically outline specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Production Area, Extrusion Departments: On or about May 31, 2017, and at times prior, specific procedures for shutting down, isolating, and securing energy sources were not developed and utilized when servicing and maintaining equipment such as, but not limited to extrusion equipment, exposing the employees to electrical and amputation hazards.
