29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Production Area - Twisting: On or before September 27, 2018, and at times prior, the employer exposed employees to struck-by and caught-in hazards associated with yarn mill activities in that the employer did not identify the person certifying that the evaluation has been performed and it did not identify the document as a certification of hazard assessment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Production Area: On or about September 27, 2018 and at times prior, the employer exposed employee to electrical hazards and amputations, in that periodic inspections of the energy control procedures were not conducted for Lock Out/ Tag Out (LOTO) including, but not limited to, Volkmann Twisting Machine #1 and Heat-set Machine #1.
