29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On the eastern side of the building where a concrete block wall was being constructed, guardrails were missing from all sides of the tubular welded frame scaffold's top platform, thereby exposing employees to approximately a 19 foot fall hazard. b. On the eastern side of the building where a brick wall was being constructed, a guardrail was missing from the front side of the hydro-mobile scaffold. An approximate 12 inch opening exposed employees to approximately a 16 foot fall hazard. RVM Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 459996, citation number 1, item number 2, and was affirmed as final order on August 12, 2012, with respect to a workplace located at 825,875 Burr Ave., Grandview Heights, Ohio 43212.
