29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Outside- Restaurant: The control switch pendant station which had damaged insulation in that the outer sheathing was pulled away from the wires on the lift controlling device. Employees were exposed to possible injury when handling the controls to operate the Skyjack Scissors Lift #SJ1113219.

29 CFR 1910.157(c)(4): The employer shall assure that portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use. Maintenance Room : A portable fire extinguisher was not maintained or inspected. The extinguisher viewing gauge showed it to be undercharged. Employees were exposed to possible injury in case of fire. Manufacturer: Ansul Sentry ABC S/N 9994682.
