29 CFR  1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.  1. Located in the back of house, a magic chef miniature refrigerator was plugged into a relocatable power tap.  2. In the employee break room, a 1000 W output Hamilton Beach microwave was plugged into a relocatable power tap instead of directly to a properly grounded outlet as per the instructions on the back of the microwave. 3. Located behind the hotel lobby desk near the printer, a relocatable power tap was affixed to the interior wall using zip ties.   Employees were exposed to electrical hazards.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Located in the shipping and receiving dock, electrical panels were obstructed by hand sanitizers, a filing cabinet, a mini fridge, and other items. Employees were exposed to an electrical hazard that could have led to injuries such as electric shock or burns from fire.

Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:  Located in the maintenance room next to the pool, a light switch did not have a faceplate. Employees are exposed to electrical hazards from exposed energized parts.

Located at Redwood Hotel and Casino, the bloodborne pathogens exposure control plan was not reviewed and updated at least annually to reflect any new or modified tasks such as the removal of needles from public areas such as restrooms. The exposure control plan had a date of 01-19-2016, rev. 4 and did not include information on how to handle the sharps containers located in the public restrooms. Employees are exposed to potential bloodborne pathogens.
