29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about June 7, 2017, an employee was exposed to fall hazards while installing shingles on a commercial roof and the employer did not ensure that the personal fall arrest system that the employee was utilizing was installed properly.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about June 7, 2017, an employee was exposed to eye injuries while using a pneumatic nail gun to install shingles on a residential roof and the employer did not ensure that appropriate eye protection was utilized. The Steinmeyer Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 976896, citation number 1, item number ,1 and was affirmed as a final order on November 7, 2014, with respect to a workplace located at the corner of College and St. Louis St. in Sparta, IL.
