29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) For employees working at 2500 Edwards Drive in Bradenton, as observed on or about June 2, 2017. Employees were installing feeder wires into a switchgear assembly. One of the employees was on top of the panel working without the use of a fall protection system exposing the eemployee to a 8 feet fall.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. a) For employees working at 2500 Edwards Drive in Bradenton, as observed on or about June 2, 2017. Employees were installing feeder wires into a switchgear assembly when an arc flash occurred as one of the ground wires contacted an energized feeder bus bar in the adjacent panel. The insulated barrier between the panels was cut out previously by employees installing the connecting bus bar.
