29 CFR 1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck(s): (a) Cosentino North America dba Cosentino Center of Denver, 12435 E 42nd Ave., Denver, CO 80239: On and before June 12, 2017 an employee walked between a granite slab that was elevated by a boom, and the forklift. This condition caused the employee's foot to be run over by the forklift. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
