29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. On or about June 10, 2017, an subcontractors employee was exposed to live electrical current of up to 27KV. The employee was subcontracted to drill three, 5 inch holes in the concrete floor of a large electrical cabinet located at 375 Victoria Road, Youngstown, Ohio 44515. The employee entered the cabinet without any form of protection.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into pysical or electrical contact with the electric power circuit. On or about June 10, 2017, a subcontractors employee was exposed to live electrical current of up to 27KV. The employee was contracted to drill three, 5 inch holes in the concrete floor of a large electrical cabinet located at 375 Victoria Road, Youngstown, Ohio 44515. The employer did not do a pre-job safety analysis to ascertain weather or not the employee could contact the live current.

29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged. On or about June 10, 2017, a subcontractors employee was exposed to live electrical current of up to 27KV. The employee was contracted to drill three, 5 inch holes in the concrete floor of a large electrical cabinet located at 375 Victoria Road, Youngstown, Ohio 44515. The electrical current could be isolated and turned off using air switches on a corresponding electrical pole. The air switches were not locked out at the lockout point at the base of the pole.
