29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103): (a) A laborer using an abrasive grinder and concrete saw to clean, widen and repair concrete cracks, joints and damaged areas, wore a half face elastomeric respirator over facial beard stubble. This exposes the employee to silica dust and particulate concrete dust.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) A portable wobble light that illuminated the work area had the electrical cord sheathing and strain relief of the extension power cord pulled away from the plug exposing the electrical wires. This exposed the employees to an electrical shock hazard.
