29 CFR 1926.152(a)(1): Only approved containers and portable tanks shall be used for storage and handling of flammable and combustible liquids. Approved safety cans or Department of Transportation approved containers shall be used for the handling and use of flammable liquids in quantities of 5 gallons or less, except that this shall not apply to those flammable liquid materials which are highly viscid (extremely hard to pour), which may be used and handled in original shipping containers. For quantities of one gallon or less, the original container may be used, for storage, use and handling of flammable liquids.  Maya Erectors, LLC project site located at 618 De Pere Rd. Denmark, WI: On and about June 14, 2017, employees were exposed to fire and/or explosion hazards while working near a 5 gallon gasoline can that was missing the self-closing portion of the spout and did not have a cap to prevent vapors from escaping.

Maya Erectors, LLC. project site located at 618 De Pere Rd. Denmark, WI: On and about June 14, 2017, employees were exposed to falls due to horizontal lifeline systems being installed without being designed and installed by a qualified person, in that:  A) The system did not include a shock absorbing device to protect the stanchions;  B) Standard wire rope clips were used for a critical application and were not installed using a torque wrench;  C) No employee on site met the criteria for a qualified person by extensive training, degree or certificate.

Denmark, WI: On and about June 14 and 21, 2017, employees were exposed to falls while erecting a steel building in that:  A) On June 14, 2017, two employees erecting a horizontal lifeline were sitting and kneeling on a steel beam without fall protection;  B) On June 21, 2017, employees were installing steel decking on the roof when personal fall arrest systems were not used and a Controlled Decking Zone was not established in accordance with 1926.760(a)(3).
