Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5. a) Utility Department, on or about and at time prior to June 20, 2017, the employer did not ensure protective equipment such as, but not limited to, Novax Class O, Type I dielectric gloves, which were available for use was periodically inspected, exposing employees to electric shock and arc flash hazards. 1.
