29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Roof over the Front Sidewalk, on the Side with the Roll-Up Door - Employees replaced a roof drain that did not function, and the employees did not use a fall protection system, exposing them to falling 14 feet to the ground below, on or about June 16, 2017. Corrected during the inspection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Roof over the Front Sidewalk, on the Side with the Roll-Up Door - Employees used a portable extension ladder to gain access to the roof to replace a roof drain that did not function. This ladder did not extend three feet above the roof (upper landing) surface, exposing the employees to the hazard of falling 14 feet to the ground below, on or about June 16, 2017. Corrected during the inspection.
