29 CFR 1926.451(g)(4)(ii): The top edge height on all suspended scaffolds where both a guardrail and a personal fall arrest system were not between 36 inches (0.9 m) and 45 inches (1.2 m) above the platform surface: Note: For supported scaffolds manufactured and placed in service before January 1, 2000, and on all suspended scaffolds where both a guardrail and a personal fall arrest system are required, the top edge height should be between 36 inches (0.9 m) and 45 a. On or about June 19, 2017, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, eighteen (18) feet six (6) inches above the ground, the end rail was not installed at the required height, thereby exposing the employee to the hazards associated with falls.
