The employer had not provided training on fall hazards as required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Lot #12 on Kylemore lane, Brandon, MS- On or about June 8, 2017, an employee was using a ladder to access the roof of a residential construction and the ladder was not extended 3 feet beyond the landing of the roof.
