29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On or about 06/19/17, on a jobsite at 205 Carolina St., Buffalo, NY:  An employee engaged in carpentry work used an extension ladder to access the top of a dormer  at the northwest corner of an apartment building. The employee was exposed to a fall of approx.  21 ft.  because  the  siderails of the extension ladder used to access the dormer were approx.  4 ft.  below  the  top of the dormer.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:       a) On or about 06/19/17, on a jobsite at 205 Carolina St., Buffalo, NY:  An employee engaged in carpentry work used an extension ladder as a workplatform  to do a carpentry work  on the  dormer  at the northwest corner of an apartment building.  The employee was  exposed to a fall of approx.  21 ft.  because the  ladder was  set on the side walk and could be displaced by persons walking by.  The ladder was not secured.         NO ABATEMENT CERTIFICATION REQUIRED
