29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:        a) On or about 07/20/17, on the southeast part of the roof on a house at 42 Richmond Ave., Buffalo, NY: Three employees engaged in roofing work on a steep roof were exposed to a fall of approx.  20 foot.   All the employees were wearing full body harnesses, but none of the employees were tethered to an anchor point. The roof pitch was 12 in 12.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On or about 07/20/17, on the south side of the house at 42 Richmond Ave., Buffalo, NY:  Employees used an extension ladder to access the roof.  The side rails of that ladder only extended approx.  18 inches above roof.         NO ABATEMENT CERTIFICATION REQUIRED
