29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Loading Dock, Door #3: Employees are exposed to electric shock/fire hazards by using a Loading Dock Light with a damaged flexible cord that has separated from the outer insulation. THE DEFENSE COMMISSARY AGENCY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 114064, CITATION 02, ITEM NUMBER 002 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 07, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT DEFENSE COMMISSARY AGENCY, 795 REEVES BOULEVARD, LEMOORE NAVAL AIR STATION, CALIFORNIA 93246.
