29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Employees were exposed to crushing/pinch point hazards while working on and around the cracking mill, and the employer did not ensure the guard on upper motor was properly affixed to the machine.  The guard on the upper motor was easily removable and exposed employee's to ingoing nip points.    (b)  Employees were exposed to crushing/pinch point hazards while working on and around the box forming machine and the employer did not ensure the machine was properly guarded.  Access panels on the box forming machine could be opened, which exposed employees to ingoing nip points and pinch point hazards, while the machine was still in operation.    The Olds Products Co. of Illinois was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1139538, citation number 1, item number 5 and was affirmed as a final order on October 5, 2016, with respect to a workplace located at10700 88th Avenue in Pleasant Prairie, WI.
