On or about June 22, 2017, at the JEA Sewage Station on Old Middleburg Road in Jacksonville, in the 5 feet and 5 inches deep trench, employees installing sewer line were exposed to a cave-in hazard in that they were not protected by an adequate protective system.  Petticoat-Schmitt Civil Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 996308, Serious citation number 01, item number 002 and was affirmed as a final order on January 12, 2015, with respect to a workplace located at 2003 CR 210, Saint Augustine, Florida.
