29 CFR 1910.178(n)(1): All traffic regulations were not observed, including authorized plant speed limits. A safe distance was not maintained approximately three truck lengths from the truck ahead, and the truck was not kept under control at all times. On or about June 22, 2017, the employer failed to ensure all powered industrial vehicles were operated in safe manner: a) a fork lift slammed into the side of a delivery truck which dented the delivery truck as it was being loaded with beverages; and b) fork lifts throughout the plant were operated at full speed when approaching other stationary and mobile fork lifts at a distance of less than one truck length away.
