29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) Bodycote Thermal Processing, Inc./Roselle, New Jersey- Warehouse and Vacuum Furnace room: Employees conducting maintenance repairs on the Vacuum Furnace System (VFS) were using Clark (GCX25) forklift which was defective. The concentration levels of carbon monoxide (CO) gas created by the Clark forklift exceeded the level of 50 parts per million (ppm). Condition occurred on or about June 14, 2017.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: a) Bodycote Thermal Processing, Inc./Roselle, New Jersey- Vacuum furnace room: A contractor, an employee of Ipsen working at the facility was exposed to carbon monoxide (CO) during maintenance activities on the vacuum furnace system. The employee was exposed to carbon monoxide (CO) during a 150 minute occupational exposure at a mean calculated 710.3 ppm CO level. The %COHb in the employee's blood reached a calculated peak level of 42.2%. The 8-hour TWA exposure was 222.0 ppm for CO, which is 4.44 times the OSHA PEL of 50 ppm for CO. Condition occurred on or about June 14, 2017.
