On or about June 21, 2017, access to exits were hindered by materials projecting into the exit route and the minimum width to the exit was not maintained.  a) Access to the exit door between the dock and office area was hindered by materials projecting into the exit route.   b) In the garage, access to the southwest door was obstructed by a pallet of materials.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    CGA P- 1 : Safe Handling of Compressed Gases 3.3.8  - Protect cylinders from any object that will produce a cut or other abrasion in the surface of the metal. Do not store cylinders near elevators or gangways, or in locations where heavy moving objects may strike or fall on them. Where caps are provided for valve protection, such caps should be kept on cylinders in storage.     On or about June 21, 2017, employees were exposed to injuries from the compressed gas cylinders that were stored unsecured next to the dock doors exposed to vehicle traffic loading and unloading at the dock.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about June 21, 2017, employees were exposed to injuries from compressed gasses, cleaning chemicals and other hazardous chemicals and the employer had not provided information and training on hazardous chemicals in their work area at the time of their initial assignment and when a new hazard was introduced. Employee training shall include:  1. The requirements of the standard. 2. Any operations in their work area where hazardous chemicals are present. 3. The location and availability of the written hazard communication program, including the required list of hazardous chemicals, and safety data sheets required by the standard. 4. The methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.) 5. The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area. 6. The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used. 7. The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information.
