29 CFR 1910.23(c)(1): (1) Rungs and steps of portable metal ladders are corrugated, knurled, dimpled, coated with skid-resistant material, or otherwise treated to minimize the possibility of slipping; a. On or about September 29, 2017, located at the Assembler on Line 1 there was a portable ladder that was used for employees to climb to remove jammed caps out of a chute conveyor. The ladder was missing its fourth rung and replaced with a section of a broom handle that was smooth with no means of skid-resistant material, thereby exposing the employee to a fall hazard of approximately four feet.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about September 26, 2017, located on the Line 1 Assembly Machine, the employer did not include on their written control of hazardous energy procedures, (A) A specific statement of intended use of the procedure, (B) Specific procedural steps for shutting down and isolating all energy sources, specific procedural steps for placement removal and transfer of lockout devices and specific procedural steps for verification that the machine's energy sources were deenergized and at a safe state, thereby exposing the employees to the unexpected startup and/or release of stored energy. The lockout/tag procedures provided for the Assembly Machine did not include the hydraulics as a source of energy, the magnitude of the pneumatics, and specific procedures for the removal of lockout devices. The procedure also included pancake devices to be used when there were no gate valves on the machine.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about September 26, 2017, employees were exposed to machine hazards associated with moving parts while removing jammed caps of the assembler on Line 1. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about June 29, 2017, located Line 2 assembler, light curtains were able to be disabled while the machine was in operation, thereby exposing the employees to the moving parts of the assembler. b. On May 22, 2017 on Line 2 assembler, an employee reached through a disabled light curtain to insert a cap on the end of a tube. The employee inadvertently cycled the machine by pressing a start button causing an amputation of a finger. c. On or about June 29, 2017, located at the Line 2 cutter, the guarding was left in the open position with interlocks disabled while the automated cutting head traversed, thereby exposing the employees to a struck-by/caught-in hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or about June 26, 2017, located on Line 2 at the hopper, employees adjusted and inspected tubes while there was an unguarded chain and sprocket that was approximately 52 inches off of the floor and 34 inches from the top step where employees would reach near the sprocket, thereby exposing the employees to a caught-in hazard.
