29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not de-energized before an employee worked on or was near to them: (a) Mechanical Air, Inc. at 1370 Grant St., Denver, CO 80203: On and before June 26, 2017, the employer did not ensure that live parts to which an employee may be exposed were not de-energized before the employee worked on or was near to them. An HVAC employee was working on the control system for a variable air volume unit without de-energizing it. The employee came into contact with the energized 208 volt circuit, and was fatally injured when he fell from a stepladder. This condition exposed the employee to electrical shock and fall hazards.

An HVAC employee was working on the control system for a variable air volume unit without de-energizing it. 29 CFR 1910.334(c)(1): Unqualified persons performed testing work on electric circuits or equipment: (a) Mechanical Air, Inc. at 1370 Grant St., Denver, CO 80203: On and before June 26, 2017, the employer allowed unqualified persons to perform testing work on electrical circuits or equipment. HVAC employee were allowed to perform voltage testing on electrical circuits without the use of appropriate safety-related work practices such as training and the use of personal protective equipment. This condition exposed the employee to electrical shock and fall hazards.
