If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used. M/V Jubilee: Employee(s) checking the for leaks were accessing the vessel without a gangway by stepping over an approximate 1-3 foot gap from the pier to the swim step.
