29 CFR 1910.212(a)(3)(ii):  Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    a) The SBJ Group Inc. at 3518 Larimer St., Denver, CO 80205:  On and before June 19, 2017, the employer did not ensure that the Accurshear hydraulic shear model # 625010, serial # 4127, was guarded at its point of operation.  An employee was cutting small metal parts, and experienced a finger-tip amputation when the hold-down clamp contacted the employee's finger.  This condition resulted in exposure to an amputation hazard at the hold-down clamp point of operation; and    b) The SBJ Group Inc. at 3518 Larimer St., Denver, CO 80205:  On and before June 19, 2017, the employer did not ensure that the hydraulic press brake #13012 was guarded at its point of operation.  Employees operated the press brake to bend metal parts without adequate machine guarding, and without an effective safe distance program.  This condition resulted in exposure to an amputation hazards.
