29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee was protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. Underbody Coating Dept., P-11 Conveyor Line. On or about June 29, 2017, employees were required to walk across a moving conveyor at two designated crossover areas, approximately two feet above floor level. At the crossover areas, the recessed conveyor chain track beneath the walking surfaces created holes (approx. 4.75" wide x 36" long x 11" deep) that were not guarded by covers, guardrails or other equivalent means to prevent employees from tripping into or stepping into them. Employees were exposed to strain/sprain injuries, contusions and fractures. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Underbody Coating Dept., P-11 Conveyor Line. On or about June 29, 2017, employees were required to walk across a moving chain conveyor at two designated crossover areas. There was no method of guarding utilized to prevent employees from becoming caught in the moving conveyor chain in the event of a trip or fall into the open conveyor track at the crossover areas. Employees were exposed to lacerations, contusions and fractures. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
