29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    Jobsite:  The employee who was working from the basket of an aerial lift was not only wearing a full body fall arrest device in a wrong manner, but also the lanyard was not attached to the basket or boom.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  Jobsite: The employee who was finishing punch list items as part of the structural steel work on the roof was not protected from fall hazards by the use of appropriate fall protection systems/devices.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Jobsite:  The employee who was working from the basket of an aerial lift without fall protection at a height approximately 45 feet above the ground were not trained to recognize the hazards associated with the type of scaffold being used.
