29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.  a) Slabaugh Construction Incorporated on or about June 29, 2017 at the address of 1530 South Danbury Road, Port Clinton, OH, 43452: The employer failed to ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential wood framing operations (i.e.; fall protection, and use of personal protective equipment).

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide.   (a) Slabaugh Construction Incorporated on or about June 29, 2017 at the address of 1530 South Danbury Road, Port Clinton, OH, 43452: The employer failed to ensure the 12in wide working platform, in which two employees were securing structural panels from, was of sufficient width. Two employees were exposed to a potential fall of up to 21.9ft to the level below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) Slabaugh Construction Incorporated on or about June 29, 2017 at the address of 1530 South Danbury Road, Port Clinton, OHIO, 43452: The employer did not ensure employees were protected from a fall while working from a scaffold.  The employees were exposed to a 21.9ft fall to the level below while securing structural panels into place. No form of fall protection was in place.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.   a) Slabaugh Construction Incorporated. on or about June 29, 2017 at the address of 1530 South Danbury Road, Port Clinton, OH, 43452: The employer did not ensure an employee's personal fall arrest system was secured (clipped-off) to a Freight Liner FL20 (Lift All Hoist) bucket truck. The exposed employee was performing wood framing activities from the aforementioned aerial lift, by guiding structural panels to be secured. The employee was exposed to a potential fall of 28ft to the level below.
