29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval:   (a) Facility, Perishable Building:  On or about June 19, 2017, an employee was operating a Barnett powered industrial truck (tugger), serial number 20-35368) and the manufactured trailer coupler was replaced with one that was not approved by the manufacturer.  In addition, two pieces of pipe were welded to the front of this tugger to hold equipment for janitorial employees.  This was also not approved by the manufacturer.
