29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a) On or about June 29, 2017, and at times prior, the employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a roll-former machine where the unexpected energizing, startup or release of stored energy could occur and cause injury, the roll-former machine was not isolated from its energy source and was not rendered inoperative. Employees were exposed to caught-in and amputation hazards while using a stepladder to reach over a vertical steel mesh guard to unjam and/or to align metal parts into the guide channels entering the roll-former without isolating the energy source on the machine.
